Complaint/Grievance Process

If you have a concern or problem, or are not satisfied with your services, Placer County Systems of Care wants to be sure your concerns are taken care of simply and quickly. There are a number of ways you can get this help:

If you have a Systems of Care case manager or caseworker you can tell them your concern and they can try to take care of the problem or help you file a grievance or appeal.

Or you can get Grievance/Appeal forms and addressed envelopes at Systems of Care offices and submit a written grievance or appeal.

Or you can get help with filing a grievance or appeal by calling one of the following:

Systems of Care Managed Care Unit at 530-886-5400

Patient's Rights Advocate at 530-886-5419

Parent/Family Representative at 530-886-5473

If you are calling long distance, you can call 800-488-4308 toll-free to be connected to the above numbers.

Your concern will be handled as quickly and simply as possible. It will be kept confidential. You will not be subject to discrimination or any other penalty for filing a grievance or appeal. You may authorize another person to file your grievance or appeal for you.
